Our Diabetes Action Plan Program is a service we offer our patients at no charge. DAPP is an individualized and specialized program for diabetics. We have a nurse case manager available to give you one on one counseling, answer questions or just offer simple support whenever you need it. Your nurse case manager will give instruction on how to best manage your diabetes so that you can enjoy life and eating without worry.
As part of our DAPP program, you will receive valuable information, support, educational materials and classes, important vaccines, even some tests and supplies – all at no charge to you!
